Factors Affecting Survival in Patients With Brain Metastasis Prognostic factors affecting the survival in patients with brain metastasis: A Retrospective study Abstract Aims: To determine long term survival and prognostic factors in patients with brain metastases who underwent whole brain radiotherapy. Introduction: Multiple brain metastases are unfortunate consequence, frequently found in patients of advanced cancer. The prognosis, even after treatment with Whole-Brain Radiation Therapy (WBRT) is poor with an average expected survival time of Materials methods: From January 2005 to August 2010, medical records of 186 patients with diagnosis of brain metastasis were analyzed. Out of these, 140 patients who received WBRT Â ± chemotherapy were included and 26 patients who did not take any treatment for brain metastasis were excluded from the study. The prognostic factors evaluated for overall survival were ECOG performance status, gender, age, number of lesions, primary tumor site, primary tumor status, extracranial metastases and chemotherapy radiotherapy. Results: The overall median survival was three months and one two year survival was 8.57% and 3.57%, respectively. The most common primary tumor site was lung 82 (44.08%) followed by breast 46 (24.73%), renal cell carcinoma 11 (5.91%) and unknown primary 11 (5.91%). The overall median survival was 3 months, maximum being 4.5 months in patients with breast cancer. and at one year survival 6/33(18.2%) in breast cancer patients (p=0.10). In this series, the patients with higher perform status (p=0.21), cancer breast (p=0.10) and solitary brain metastasis (p=0.0003) with primary tumor controlled (p=0.14) had better survival. It is devastating both to patients and their families. Progression of brain metastases may cause headache, nausea, vomiting, neurological deficits, cognitive decline, delirium and eventually death. Patients with brain metastases present dilemma for palliative health care professionals in terms of whether to proceed with whole brain radiotherapy or hospice placement. Metastasis to the brain occurs in approximately 20% of the patients with limited survival and worse quality of life. [1] Glucocorticoids and whole brain radiotherapy (WBRT) has been the mainstay of treatment while craniotomy for tumor resection has been the standard local treatment for solitary lesion. The median survival of untreated patients is approximately one month, 1.6 months in patients treated with steroids only, 3.6 months in patients treated with radiotherapy and 8.9 months in patients treated with neurosurgery followed by radiotherapy. [2, 3] So me of the strong prognostic factors for survival are: performance status, response to steroids and status of systemic disease. The main goal of WBRT is to improve neurologic deficits caused by the metastases and surrounding edema and to prevent any further deterioration of the neurologic function. The extent of improvement after WBRT is directly related to the time from diagnosis to radiation therapy and early treatment is generally associated with a better outcome. [4, 5] The overall response rate to WBRT ranges from 50-85% in various studies. Traditionally, surgical resection has been offered rarely to patients with multiple metastases because of resection related excessive morbidity. The majority of patients who achieved control of cranial metastasis died from progressive extracranial disease whereas the cause of death in most of the cases is due to CNS disease in patients with recurrent brain metastases. [4, 6] In this study, the prognostic factors were evaluated for survival in patients with diagnosis of brain metastasis who receive WBRT. Materials Methods The records of 186 patients with brain metastases in between January 2005 to August 2010 were analyzed retrospectively. Out of these, 46 patients not willing for radiotherapy and who chose only best supportive care were not included in the study. All the remaining 140 patients were planned WBRT after starting dexamethasone, mannitol and other supportive treatment. With diagnosis of brain metastasis, the following variables were analyzed for survival: ECOG performance status, gender, age, number of brain lesions, primary tumor site (Table 2), extra cranial metastases, treatment of primary disease and radiotherapy (Table-1). The survival time was considered as time between diagnosis of brain metastasis and last follow up or recorded death. Brain metastases were detected by computed tomography (CT) or magnetic resonance image (MRI) scan. All patients were treated with WBRT on telecobalt units Therateron 780 C and 780 E. The WBRT was given by bilateral fields. The total dose was 30-36 Gy with a median of 30 Gy delivered in two weeks, five fractions per week 3 Gy per fraction. The supportive care (dexamethasone mannitol) was started at the beginning of treatment and continued during radiotherapy. Chemotherapy was administered to the patients with good performance status and progressive systemic disease after WBRT. All statistical analyses were performed using SPSS for windows, version 20.0. Results One hundred forty patients were analyzed who completed WBRT and came for follow up. The survival results were computed by recorded deaths (97 cases) or last follow up (43 cases). The overall median survival was 3 months, and the one two year survival was 12 (8.57%) and 5 (3.57%). One patient of carcinoma breast (primary under control) with brain metastasis was alive at the time of this analysis with survival time of 4.2 years. Those patients who had solitary brain metastasis (p=0.0003), high ECOG performance status (p=0.21), controlled extracranial disease (p=0.14) and breast carcinoma (p=0.10) had better survival (Table 2 3). The single most significant prognostic factor associated with better survival was solitary brain metastasis (p=0.0003). Discussion With gradual improvements in the care of cancer patients, longer survival is expected even in patients with metastatic disease. In this study, patients with brain metastases who received WBRT alone or WBRT followed by chemotherapy were evaluated. Studies of ultra rapid fractionated WBRT (10 Gy in 1 fraction, 12 Gy in 2 fractions, 15 Gy in 2 fractions over 3 days) as carried out by Radiation Therapy Oncology Group (RTOG) and other investigators showed a possible increased risk of herniation and death within a few days after treatment and are generally avoided. Likewise, no advantage was seen with extended fractionation (50Gy in 20 fractions or 54.4Gy at 1.6Gy twice daily) compared to the more commonly prescribed 30Gy in 10 fractions. [6, 7, 8, 9] Regimens using 10 or fewer fractions are used in patients with poor prognosis, since such patients are not expected to live long enough to experience serious side effects. The institutional protocol followed in our patients is 30 Gy in10 fractions, 3 Gy per fraction but in patients with good general condition and primary disease under control having solitary brain metastasis, the dose was escalated by 6 Gy to give total tumor dose of 36 Gy. The end point of this study was to evaluate the different prognostic factors related with overall survival in patients with brain metastasis. The prognostic factors associated with better survival were solitary metastasis (p=0.0003), breast carcinoma (p=0.10), female sex (p=0.12), primary under control (p=0.14), higher ECOG performance status (p=0.21). These prognostic factors have also showed better survival in other studies. [7, 10, 11, 12, 13] Out of above mentioned prognostic factors only solitary brain metastasis was statistically significant (p=0.0003); other factors could not show statistical significance which may be due to small number of study sample. Lutterbach et al reported overall median survival of 3.4 months, two yea and three years survival were 5.6% (n=48) and 2.9% (n=25), respectively. [14] Survival of two years or more was observed in RTOG recursive partitioning analysis (RPA) class 1 2 patients. Within both classes, survival was significantly better for patients with a single brain metastasis compared with those having multiple brain metastases. In our study, the overall median survival was 3 months, and the one two year survival was 8.57% (n=12) and 3.57% (n=5), respectively and solitary brain metastases survival was significantly better than multiple metastasis (p=0.0003). There is small difference in two years survival in both studies because in our study, no patient received Stereotactic radiosurgery (SRS); however, a larger recently published trial (RTOG 95-08) provides compelling evidence for the use of SRS boost following WBRT in patients with newly diagnosed one to three brain metastases. [15] In other recent studies, the role of WBRT following definitive treatment (surgery or SRS) of one to three metastases was most extensively evaluated in a trial conducted by the European Organization for Research and Treatment of Cancer (EORTC 22952-26001) which was presented at the American Society of Clinical Oncology (ASCO) meeting in 2009 . In that trial, 359 patients with one to three brain metastases were randomly assigned to WBRT or observation following definitive treatment of their metastases with either SRS (n = 199) or surgery (n = 160). Despite the better control of the brain metastases, overall survival was virtually the same following WBRT (median 10.7 and 10.9 months). [16] Pease NJ et al showed that patient’s survival with WBRT was increased by an additional three to seven months from unselected group (three to six months) if they are in the high performance status group. [17] For those in poor performance status groups, there was no overall survival benefit. In our study, ECOG performance status 1 2 had better overall median survival (3.4 months) and one year survival (7.85%) as compared to ECOG status 3 and 4 (p=0.21). Lagerwaaed FJ et al reported that lesser systemic tumor activity was showed better median survival ranges from 6.6 months for the ‘none’ group (no extracranial disease) to 3.4 months in the ‘limited’ group and 2.4 months in the ‘extensive’ group (primary uncontrolled other systemic metastasis). [18] In our study, overall survival was 2.2 months 4 months in active primary disease and controlled primary with or without systemic metastases respectively. In our study, female showed better survival than male patients (p=0.12). This may be due to all breast cancer cases were females and breast cases showed better survival than others. Other studies did not show gender related survival difference. Conclusion WBRT continues to be an efficacious treatment in the management of brain metastasis. Despite the use of WBRT, outcomes are poor and efforts should be made to incorporate multimodality approaches including surgery, radiosurgery, chemotherapy and radiotherapy sensitizers to improve survival mainly in patients with single metastasis, good performance status and extra cranial disease controlled. Relations of Television Viewing and Reading Essay

A lot of studies concerning television viewing impacts on students’ academic performance have been made, but there are still questions related to this that are needed to be answered. Concentration Deterioration hypothesis – television’s rapid changes in context and pacing may cause children’s inability to concentrate on a task. 4. Reading Depreciation hypothesis – Students’ motivation in investing energy in learning may diminish due to pleasant experiences with television. After discussing these hypotheses, the researchers enlighten up the positive side by referring to facilitation hypothesis. This hypothesis views television as a facilitator to one’s reading development. Then, other factors affecting children’s intellectual ability such as IQ and SES were discussed. The results also showed that viewing type/duartion (light, medium, and heavy) is related to their reading progress over time. Light and medium viewers have faster reading progress compared to the heavy viewers. If these were included in their study, then these two hypotheses would be enlightened and there would be more contribution to the body knowledge of television viewing. Methods were expounded all throughout the paper but not in a concise manner. There were cases wherein concepts related to methods were discussed more than once. It would be more convenient to readers if methodological processes were summed up into one section. Through this, it would be easier to locate the overall procedure conducted in the study. Materials, equipment, and procedure were also not explained well in the article. Descriptions are needed so that readers would be able to understand the context meaning of certain words included in the paper. For example, the study did not describe what general audience entertainment programs are and how they differ from educational programs. There was a gray line between those two. There are educational programs which could be entertainment programs, too. There is a need for clarifications of descriptions. This very crucial in the conduction of study because the parents, who will record the TV programs genre watched by their children, must be informed well so that they would be able to classify the genre of certain TV programs. Yes, the researchers listed the title of educational programs in their study, but how about the audience entertainment programs? Also, descriptions are important in this for future reference.

Angela was married twice. Her first attempt was made to Ulrich Merkel, the physician, in 1977. However, it ended up with divorce five years after. There are several reasons why teenagers run away from home, normally these reasons are related to conditions in the families, bad influence of their friends and financial problem. The first cause of running away is conditions in the family. One of the reasons is because they come from broken homes where their parents are divorced. They do not get enough love and care from their parents. As a result, they will go and find love and care elsewhere. Some parents have the habit of quarrelling in front of their children. The children feel ashamed of their parents and cannot stand this kind of environment. In the end they run away from home. Some parents are too strict with their children. Since alcohol is used to celebrate and have fun, teens that abuse alcohol most likely has no idea of the affect alcohol and its abuse can have on their bodies. If they find bad friends that influence of them, they begin to offer drinking and in the end teens used to drink and become an alcoholics. Then finish their life in the street. Also teens may become drug addict, which in turn lead to depression and suicide. The most common reason why teenagers use drug is to relieve stress. Many of them have to suffer stress from family condition. One of the explanations is that drugs users have a poor family life. The majority of drug users have had an unhappy childhood which includes cruel punishment and parental neglect. Many teenagers become a fraudster and a thief when the run away from home. Thus the undergrowths, which are associated with bad friends, began to becoming fraudsters or steal on the street. To steal just because they didnt want to pay for something even though theyre capable then its wrong. These people should be punished in order to learn that they were wrong for stealing. But these teenagers who have home and family, who must study, help parents, they should not steal. But others teenagers find not good pay jobs, such as water in cheap cafes, cleaner, job in car washing, janitor and so on. They need the money, but they have not opportunity and choice and they have to work in such jobs as these. They have not education and no one can help them with good job. Every teenager who run away from home, have made a bad decision. They got themselves under the pressures that they felt the need to escape from. Instead of facing their problem and solving it, they chose to run from it. When they have the right decision to change some of the things that may be going on in their lives, the pressure lessens, and there is no more need for them to escape. If they learn to solve their problem and their parents will help them to do so, without cursing and impose their goals and solutions, then the escape of teenagers reduced many times. In conclusion, there are so many teenagers who run away from their families. There are many reasons for it, but one solution parents must be more careful, understanding and loving. Patients discharge to their homes within five days (Saczynski et al., 2010), and the progress of healing after discharge demands an effective care planning, particularly, those who are newly diagnosed with IHD. After discharge patients encounter a challenge time (Eshah Bond, 2009), and life style changes include, adherence with eating heart-healthy diet, adherence with regular physical activity, and adherence with medication have ascertained by American Herat Association and considered from moderate and strong evidence (Eckel et al., 2013). adherence with these recommendations are associated with decrease readmission and mortality rate (Heran et al., 2011) , however few people adhere with recommended guidelines (Martin, Williams, Haskard, DiMatteo, 2005). Non adherence, one of the most important obstacle for successful treatment, is a widespread health problem that threat the health and cause a valuable economical burden as well (Martin et al., 2005). Non adherence to healthy lifestyle including eating unhealthy diet, physical inactivity, smoking, and non compliance with medication are known to increase the development and progression of IHD (Danaei et al., 2009). On the other direction adherence with healthy lifestyle would decrease the burden of IHD (Chiuve, McCullough, Sacks, Rimm, 2006). Medication adherence refers to whether patients take their medications as prescribed, as well as whether they continue to take a prescribed medication (Ho, Bryson, Rumsfeld, 2009). Medication non adherence is a major public health problem (Desai Choudhry, 2013). The immediate time after discharge is a high risk period for non adherence (Baroletti DellOrfano, 2010), in which 24% of patients dont adhere to their prescribed medication (Jackevicius, Li, Tu, 2008). After six weeks of discharge one forth of patients didnt adhere to prescribed medications (Mathews et al., 2012) and 80% on the long term (DiMatteo, 2004). Non adherence to medication lead to progression of the disease, increase readmission, increase mortality rate, and increase health care cost Smoking is so widespread (WHO,2007), and one of the ten strongest risk factor of IHD (Goff et al., 2013), although non adherence with healthy diet, medication, inactivity all are attributing risk factors for development and progression of IHD, smoking has a significant effect of all risk factor (CDC). Smokers have double to four times to develop IHD than non-smokers (CDC). However quit smoking is the single most effective measure to prevent IHD (Goff et al., 2013). A strong evidence exist about the casual relationship between diet and IHD (Mente, de Koning, Shannon, Anand, 2009). Eating unhealthy diet lead to increase blood cholesterol level, developing of hypertension and diabetes mellitus, obesity, and eventually metabolic syndrome which all are modifiable risk factors for IHD (Goff et al., 2013). However eating vegetables, nuts and mono-saturated fatty acid are among protective habits for prevention of IHD progression (Mente et al., 2009). Non adherence to physical activity in different culture and societies are common (Rodrigues, Joà £o, Gallani, Cornà ©lio, Alexandre, 2013). The proportion of adults who meet the recommended guidelines of American Heart Association (AHA) for regular physical activity has reduced over time (Roger et al., 2012). A recent meta analysis has been shown that an inverse relationship exist between physical activity and increasing risk of IHD; those patients who didnt do physical activity are 10-20 more risky than who do moderate physical activity and 20-30 more risky than who do high physical activity (Li Siegrist, 2012). To decrease the effect of IHD and prevent its progression secondary prevention programs done and rehabilitation centers are found, However few people attend these programs regularly (Bjarnason-Wehrens et al., 2010). Many obstacles hinder the attendance of these programs include logistic barriers like transportation difficulties, financial cost, and embarrassment of attendance (Neubeck et al., 2012). So more feasible, economical, and provide privacy for patient method is required as alternative. Tele-health, which define as the use of different type of modern information and technology to contribute t clinical support and to improve health (WHO,2009), is more economical, feasible, and provide the patients privacy. The use of mobile is growing faster and faster, and many patients have mobiles (Deng, 2013). Many studies done using technology to improve adherence among patients, especially Short Message System (SMS) in high technologic counties (Dale et al., 2014; Khonsari et al., 2014). To my knowledge this is the first study done to assess the effect of use of telehealth in a less technology-dependent countries. So the purpose of the study is: examine the effect of short message system (SMS) on medication, healthy diet, smoking cessation and physical activity adherence among IHD patients. Research hypotheses: patients who will receive reminder message will be more adherent to medication, healthy diet , smoking cessation and physical activity than those who will not. Common Law Concepts Essay -- Urban Development

CONCEPT 1 – COMPENSATION AS A WAY TO REDUCE NUISANCE (Hogan & Edwards, 2003) states the idea of compensation in Common Law is to protect the environment as it amplifies the need for organisations and individuals within a society to prevent nuisance. Furthermore, compensation concept has raised conflict between environmental concerns and restricts the property holder’s rights to fully utilize their resources. Nevertheless, the idea of compensation can also be seen as an ethical guideline for the society, of which if any laws were to be broken, for example harming the environment, appropriate punishments will be conducted. Therefore, the act of compensation may lead to a greater cause in the long run for society to reduce nuisance. To further the statement, an example of compensation having positive justification is when a British oil company British Petroleum (BP) created a large oil spill off the Gulf of Mexico. According to (BBC, 2012) BP faces charges by the United States Government and has to compensate over USD7.5 billion to the victims. Furthermore, BP was responsible to ensure the cleaning of their spillage. Prior to the event, BP reconstructed their health and safety policy by ensuring zero tolerance to callous mistakes for future shipments. This example indicates the establishment of compensation and the involvements in monetary value is of significance and therefore, discourages organisations and individuals from harming the environment in the long run. This is supported by (Reardon, 2012) â€Å"lawsuits have helped push industry to make changes in how they do business.† In other words, compensation acts as an effective way to regulate and teach property holders to refrain from damaging the en vironment. Life; A Work In Progress The average person has a grand total of 2 billion seconds to live out their existance. For those of us lucky enough to live out this time in relative health, or to exceed it, shouldn't we be trying for something more? It ends, people. Life ends. There is coming a time when your heart stops beating and you lie dead and cold on the floor somewhere--and that is it. No redo's, no timeouts, no second chances. Only the bittersweet what-ifs that you will have plagued yourself with for the remaining moments of life. 2 billion seconds doesn't seem nearly as long as my life feels, and I am still a kid. And yet we waste time bickering over the television, fighting over the computer, and resenting each-other over petty quarrels. A saying comes to mind when you consider how sad people can be; when you honestly consider that they would throw their time away on such things. It is a classic case of not being able to see the forest, through all the trees. When infact, those trees that you are so desperately trying to see around are the forest. You are trying to get past that next tree and then, then the forest will be in plain site. Only, it isn't there, and one more tree is in the way. First the tree of graduation is in your way and soon the tree of college jumps behind it. The tree of mediocre jobs blocks your view for years as you try and see around it it. The tree of midlife, or of retirement get in your way as you try and see the trees of contentment, happiness and fulfillment that affirm that 'yes, you have made a difference', and that when your two billionth second chimes to a close, and your eyes glaze over, you will not have been forgotten. That is all we want, people. That is it. One simple goal that will push every American until they die. That one goal of having meaning. Trying to hold on to the fact that, "if I make a difference in someone else's life, then mine will have mattered." So the question is, if making a difference to someone else's life is so important, and they are trying for the same goal you are, then why isn't making a difference in your own life just as important.
